If you are interested (and have not already filled out the survey), please e-mail me with the following info so that I can put together another dance program that will fit your needs.

• Age of your child
• Type of dance interested in: Ballet, Tap, Jazz, Hip-Hop, Other
• Best days/time of day to attend class
• Any additional info you would like to provide
